Abraham’s Bosom Care LLC provides 24-hour Customized Assisted Living Services for people on Cadi and Elderly waivers in our single-family home setting with private rooms.

Abraham’s Bosom Care LLC is a Home Care/ Assisted Living company with a holistic approach.  We work in partnership with our clients, their family, and their healthcare team to determine an appropriate level of service.  A personalized care plan aimed at meeting our client’s unique needs is developed to help the client live comfortably and safely. Our two facilities are located close to the Metro bus line and a walking distance to SunRay Shopping Center in Saint Paul.  Our Tower Street home has a handicap accessible ramp, and bathrooms fitted with handrails. Our home environment is very nice, clean, bright, and with a good floor plan. We strive to maintain an atmosphere that promotes social engagement, good health, and overall wellness.

Our Services:

• General health assessments and care coordination by RN
• Registered nurse and LPN support 24/7
• Assistance with mental health cognitive or behavioral management
• Medication administration assistance (includes set-up, storage, administration, record keeping, and diabetic/Injectable medication management)
• 24 hours supervision by trained staff
• Multilingual staff (English/Spanish/French)
• Intentional every 15 minutes routine checks
• Scheduled personal laundry for our clients, and daily housekeeping.
• Food menu system with flexibility that gives our clients variety of choices.
• Healthy snacks available throughout the day
• Provide hands-on personal care activities of daily living (ADLs) such as bathing, grooming, dressing, eating, walking, transferring, and more
• Assist in arranging and accompanying clients to doctors/social service appointments as needed
• Assist in arranging transportation to outside appointments as needed
• Assist with ambulation and exercise (Activities includes Game night, Movie night, Park walks, Community activities)
