We provide an individualized and coordinated program of activities. Services are directed at maintaining or improving a person’s ability to self-care. This includes health, social and nutritional services. Services are directed toward the achievement of specific outcomes identified after assessment in a service plan. Provide needed support for an older adult or person with disabilities who has a functional limitation and needs supervised care outside of his or her residence during the day.

We also Have registered Nurse, Dietitian and Physical Therapist at our location to help educate our clients.
