Our fully wheelchair-accessible home is thoughtfully designed with features like ramps, walk-in showers, and grab bars to support safety and ease of mobility.

We serve adults with various health concerns, helping with daily living activities such as bathing, dressing, grooming, medication management, and more. Our trained care team bring over 10 years of experience in both medical and home care settings, ensuring that every resident receives the personal attention they need.

We currently have 2 immediate openings for private rooms in our 24-hour Customized Living home.

Who We Care For
Azalea Homes proudly serves adults who are navigating a variety of physical, mental, or health-related challenges.

We welcome individuals with:

Mental and emotional health diagnoses, including depression, anxiety, bipolar disorder, schizophrenia, and other behavioral health needs
Functional limitations due to age, chronic illness, or disability
Complex or chronic medical conditions that require ongoing care coordination and support
Recovery needs following hospitalizations, surgeries, or treatments such as chemotherapy
Residents needing long-term support in a residential, non-institutional setting
