We provide supportive Adult Foster Care services through Thomas Allen and welcome referrals for a wide range of client needs, including:

Services Provided

Our home provides a safe, structured, and supportive environment focused on dignity, independence, safety, and individualized care.

Services include:

• 24-hour supervised care and support
• Medication reminders, assistance, and monitoring
• Assistance with activities of daily living (ADLs)
• Personal hygiene and personal care support
• Meal preparation, feeding assistance, and nutritional support
• Housekeeping and laundry
• Transportation to medical appointments and community activities
• Assistance attending therapy, day programs, and vocational services
• Mental health support and a structured environment
• Behavioral support, redirection, and safety monitoring
• Crisis support
• Community integration and social engagement
• Support with independent living skills
• Coordination with case managers, guardians, and healthcare providers
• Flexible admissions and transition support
• Individualized care based on client needs and goals

Transportation

Transportation assistance is available for medical appointments, therapy, day programs, community activities, and other approved client needs.
