CCP Vocational Transition Services



Community Connections Partnership (CCP) is a non-profit organization in the Minneapolis/St. Paul metro area providing support to people with disabilities. We specialize in the areas of employment, skill development, healthy lifestyles, community supports, and recreation opportunities. Our dedicated staff are committed to uncovering each person’s potential and supporting them step by step in their growth and development. We work in a partnership with the people we support and their team members to achieve the most meaningful life possible.


All of Community Connection Partnership supports are open to adults with intellectual (developmental) disabilities and/or mental illness. Many of the people we serve also need support with challenges such as autism spectrum disorder, severe and persistent mental illness, as well as chronic illness or physical impairments. No matter what a person’s diagnosis needs are, we believe everyone has something to contribute and seek to create a supportive environment where this can happen.

Funding Sources: Our supports are available to people living in the Twin Cities metro area who receive funding through Private Pay, Medical Assistance, County, Home and Community Based Waivers, including Brain Injury (BI), Community Alternatives for Disabled Individuals, Developmental Disabilities (DD).

CCP provides services from five primary locations in the metropolitan Twin Cities area. They are located in Minneapolis, Saint Louis Park, Plymouth, and Brooklyn Park. The locations all have a skilled and experienced on-site manager to maintain our organization’s commitment to quality individualized and innovative person centered services. Each site has a variety of community employment, both individually and in teams.


People come to our sites from all over the Twin Cities area. Community Connections Partnership Employment Consultants provide transportation to and from the sites both in minivans and personal vehicles. This helps to minimize the difficulty of transitioning from home to work and vice versa that can occur for some individuals.


The Garfield site is located just off the Lake-Lyndale intersection in the diverse community of Uptown Minneapolis. People at Garfield have formed positive relationships with nearby businesses and services. Garfield typically features a staffing ratio of approximately 1:4 or 1:6*. When possible, combination services with CCP Works! are available at this site. This location is on the Metro Transit line, which is utilized by some of the people who attend.

The Gorham site is located in St. Louis Park just east of Highway 7 and Louisiana Avenue. People at Gorham are often in need of more medical and/or behavioral support. This site features a sensory room and is located near parks and a library. With a 1:2 staffing ratio, Employment Consultants are able to offer greater community participation, health and wellness services, and skill development and supports. Most transportation is done in minivans with two staff and four participants.

The Plymouth site is located on Annapolis Lane between Highway 55 and Rockford Road (Cty Rd 42). There is a access to French Park, the Plymouth library, numerous walking trails, and local businesses. Plymouth typically features a staffing ratio of approximately 1:4 or 1:6*. The Medicine Lake site is located off of Highway 169 and 36th Avenue North. Medicine Lake was developed in order to offer services for those who are aging. Their focus may no longer be vocational, but instead on maintaining and increasing skills in all areas of life. CCP staff work hard to promote community involvement, health and wellness including regular exercise opportunities, cooking, crafts, leisure and games, etc. Medicine Lake typically features a staffing ratio of approximately 1:4 or 1:6*.
