Many people with disabilities need extra support in their daily lives. Connections provides that support by teaching independent living skills, assisting with employment needs, and helping people access available community resources. A CARF Three-Year Accreditation was awarded to Connections of Moorhead, Inc. in July 2014 for Community Employment Services: Employment Supports and Job Development. This accreditation decision represents the highest level of accreditation that can be awarded to an organization and shows the organization’s substantial conformance to the CARF standards. Our organization went through a rigorous peer review process that demonstrated to a team of surveyors during an on-site visit that we are committed to offering programs and services that are measurable, accountable and of the highest quality.
Adult Foster Care is a living arrangement that provides food, lodging, supervision, and household services in a family style setting. This service provides a 24-hour plan of care.
Residents in the foster home work on skills to maintain their home such as cleaning, cooking, laundry, menu planning, and shopping. The program encourages residents to be active members of our community through recreation and participation. Our program allows individuals with disabilities to form meaningful relationships with others and promotes a connection to family.
It gives people a chance to explore their individuality by assisting them to live up to their full potential.
The independent living programs provide individuals with the minimal supports they need to live on their own. Besides supporting individuals with basic living skills, the program provides emotional support and encouragement for decision making. The individual and their team will determine the number of service hours received each month according to each individual’s specific goals and circumstances. The service hours will be spent with the individual’s direct care staff to achieve the desired goals, such as:
