Provides 24-Hour Customized living services in a group home environment.

Assist with meals and home management:  Light housekeeping, heavy housekeeping, personal laundry, linens laundry, shopping, meal planning and preparation.

Assist with socialization – companion services, eating out, social outings, movie theaters, and choice of weekly and daily activities.

Assist with transportation – take clients to non-medical transportation, take client to personal and group activities in the community, arrange transportation for medical appointments.

Assist with activities of daily living – personal care including dressing, undressing, clothing selections, grooming, bathing, continence care, positioning, eating, walking, wheeling, transferring, dental / denture care, and toileting.

Assist with medication management – medication administration, assistance with self-administration, verbal or visual medication reminders, medication setup and monitoring, refill coordination, and more.

Diabetes and pre-diabetes management: Blood glucose testing and monitoring, follow diabetes diet plans, client education, continuous coordination of care and communication with client’s healthcare teams, assist with light physical exercises as ordered.

Renal management: Follow renal diet, assess of access site, coordinate dialysis, monitor input and output, monitor fluid intake, monitor weight (dry weight and weight gain), coordinate care with client’s healthcare team.

Cardiac management: Follow special diets: cardiac, dash diets, other diet; monitor blood pressure, monitor fluid intakes and outputs, monitor weight, and coordinate care with client’s healthcare team.

Other Complex health issues: Brain injury, Demential / Alzheimer’s, ALS, Myasthenia gravis and others.

Monitor other procedures as delegated by the RN and safety supervision.

Mental Health Management – provide ongoing behavior management and monitoring, dementia and Alzheimer’s disease care, Traumatic Brain Injury, and other cognitive care.

Services are provided by direct care support personnel (DSP) under the direct supervision of a registered nurse. Desnol staff are well trained and competent to provide quality services that help our resident to increase their quality of live regardless of their disabilities.
