Evergreen Home Healthcare Services is a licensed 5-bed assisted living home in Coon Rapids, Minnesota, offering personalized care in a comfortable residential setting. Our small-home model allows residents to receive individualized support, familiar routines, responsive staff attention, and a more personal living environment than a large institutional setting.
Services may include 24-hour awake staffing, medication management and administration, assistance with activities of daily living and instrumental activities of daily living, mobility and transfer support, behavioral and mental health support, RN assessment and oversight, health monitoring, meals and snacks, housekeeping, laundry, activities, transportation coordination, and hospice coordination based on each resident’s assessed needs and service plan.
Evergreen serves adults who may benefit from support related to physical limitations, chronic health conditions, memory changes, mental health needs, behavioral support needs, medication management, and assistance with daily living.
Funding options accepted include Private Pay, Long-Term Care Insurance, CADI Waiver, Brain Injury (BI) Waiver, and Elderly Waiver, subject to eligibility, assessment, service authorization, and admission criteria.
We welcome referrals from case managers, social workers, hospitals, rehabilitation and skilled nursing facilities, county teams, guardians, families, home health agencies, hospice providers, and other care professionals.
