The Fraser Home-Based Services program offers a variety of service options, many of which can be combined to best meet the needs of each client and his/her family. Services are implemented by trained Direct Support Professionals and supervised by qualified coordinators. They are provided on an hourly, daily or weekend schedule, depending on need and service authorization.

Fraser Home-Based services offer four different support options:

  • Respite Care
  • Individualized Home Supports with Training
  • Individualized Home Supports with Family Training
  • Individualized Home Supports Without Training

Respite Care

Fraser currently only offers In-Home Respite Care.  This service is provided to individuals whose primary caregiver is going to be absent or needing relief from their caregiver duties for short term periods.  Individuals need to reside in the same home as their primary caregiver.  Example supports include assistance with daily tasks, companionship, meal preparation, assistance with personal cares, and light housekeeping tasks.

Individualized Home Supports (IHS) with Training

This service is for adults who need support and training in at least one of the community living service categories.  The individual must reside in their own home or family home.

Individualized Home Supports (IHS) with Family Training

This service is for adults or children and the individual must reside in their family’s home.  IHS With Family Training provides the person and/or their family with both support and training in at least one of the community living service categories.  Families will learn how to more adequately care for and support the individual to help maintain the person’s ability to live and thrive in the home.

Individualized Home Supports (IHS) Without Training

This service is available for both adults and children who need support, assistance, and/or supervision in at least one of the community living service categories.  Individuals must reside in their own home or family home.

Community living service categories and example support options:

Community Participation

  • Increasing skills for safety and awareness when out in the community
  • Meeting transportation needs
  • Accessing and using community resources
  • Interpersonal communications skills
  • Finding and planning activities for leisure, recreation, and socialization
  • Building skills to navigate the community safely, etc.

Health, safety and wellness

  • Assistance arranging health care, meetings, and appointments
  • Prompts, guidance, direct supervision, training or instructional support to complete self-care tasks
  • Help creating plan to reach health, safety, and wellness goals

Household management

  • Prompts, guidance, direct supervision, training or instructional support to help budget, make personal-needs purchases, and manage money
  • Skill building and increasing awareness of household safety
  • Support and Advocacy as a tenant
  • Prompts, guidance, direct supervision, training or instructional support to complete household chores and routine maintenance tasks
  • Prompts, guidance, direct supervision, training or instructional support to meal plan, cook, and make healthy lifestyle choices

Adaptive Skills

  • Skill building for crisis prevention, support strategies to be self-sufficient, and problem solving
  • Support and training to implement positive support strategies, increasing positive behavior, and reducing or eliminating challenging behaviors
  • Assistance acquiring functional skills related to sensory and motor development

Funding Options for Fraser Home-Based services include:

  • Brain Injury (BI)
  • Community Access for Disability Inclusion (CADI)
  • Developmental Disabilities (DD)

Interested individuals, family members, or providers can fill out the Assessment Forms linked below.  Fraser’s Home-Based Program Team will review and reach out with eligibility information, questions, or any concerns.  If eligible, individuals will be placed on our interest list and matched with a staff person who will work with you to assess and develop a service plan that meets your needs.
