Horizon Assisted Living Inc. is a licensed 5-bed assisted living home in South Minneapolis providing 24-hour customized living services for adults 55+ on CADI Waiver, BI Waiver, and Elderly Waiver.
We offer a small, home-like setting with private units and awake staff available 24/7. Services include assistance with ADLs, medication management, mobility support, specialized diets, and coordination with healthcare providers.
We work closely with clients, case managers and families to ensure smooth transitions and are experienced with clients coming from hospitals, skilled nursing facilities, and community placements.
