We provide nursing care and related services that would take care of clients’ needs in the comfort of their home or any familiar environment that they choose. We strive to make it possible for clients to live as comfortably and as independently as possible. We have well-trained and efficient staff that work as a team in conjunction with our clients, their family member and physician or primary care providers in offering specialized care and attention in the comfort of your own home.
- Our registered nurses provide a range of home health care services to individuals in their homes. The services are available to meet the specific needs of each care receiver are coordinated by an individualized plan of care developed by LHC’s Registered Nurse in conjunction with clients and their physician. Registered Nurses are responsible for coordinating, facilitating, and providing nursing care as part of the health team. These include comprehensive health assessment, monitoring of clients’ condition, health promotion and prevention, ventilation and tracheostomy services, ostomy care and management, enteral feeding management, medication management, administration and maintenance of catheters, and IV infusion therapy
Licensed Practical Nurse
- The Licensed Practical Nurse provides skilled nursing care under the supervision of the Registered Nurse. The Licensed Practical Nurse is responsible and accountable for making decisions that are based on the individual’s custom experience and educational preparation.
Home Health Aide
- The home health aide is a paraprofessional member of the home care team who works directly under the supervision of the Registered Nurse and performs various services for the clients, the majority of which are activities of daily living. The home health aide is assigned to assist in a manner that will promote a quality and safe environment for the client.
