Perfection Health Care Services, Inc is licensed as an Assisted Living facility from Minnesota Department of Health and is enrolled with the DHS Customized Living program.
What do we provide?
We provide waiver services in Minnesota, in residential four to five-bedroom single family homes. We take all kinds of clients/residents. There is no sharing of rooms. Every resident gets his/her own room with queen-sized bed that comes with its brand-new bed sheets, a dresser, a stand, a huge closet, and a Roku Tv which is connected with our highspeed Comcast Business Wifi internet.
We provide both Licensed Practical Nursing (LPN), and Registered Nursing (RN) services. We also provide medication management, medication administration, medication set-ups, and medication reminders. We make sure that we order monthly medication in advance. We deal with Geritom Pharmacy for all medications which delivers it to our doorsteps. We check both client medications and medication supplies on daily basis. We keep client medications in a secured and locked steel cabinet inside a secured and locked office. In addition to that all we but a metal box in the metal cabinet which is also inside the office for controlled medications. We keep al laundry detergents inside the office for residents’ safety. We have a 24/7 awake staff, and ADT security system at all our doors for the safety and security of our residents and peace of mind for their caregivers, loved ones, and other responsible parties. We do make all the medical appoints, drive them there, and be there with them in most cases. We provide our residents with three meals a day, plus snacks. We do all the daily housekeeping. The laundry is twice a week one during the week, and the other on the weekends, and as needed too. We provide Xfinity Cable TV in the main common areas of every floor, and Roku TV in the client rooms.
What are the documents we need?
If interested, we will first need the client’s information such: As the MNChoice, CSSP, CSP, face sheet, summary and so on. We will also need the contact details of the client’s rep payee and if the client has any spend downs, co-pays …….and so on.
Note: We are not a GRH certified yet, but we are working hard to become one.
As we always say: The fast the client’s case manager(s) and Rep Payee move, the faster we will move.
Note: As a principle, we allow clients to move in, if and only if, we receive an approved/guaranteed 24/7 Customized Living Daily Rate, and a “Service Agreement Letter” is in the Minutes System (MN-ITS), and a Rep Payee in place with agreed Room and Board (R&B) fees.
In Short as soon as everything is approved from all sides involved, the client can move in.

Below are the answers to some common questions that we are asked:
Do you accept waivers such as, Elderly and CADI waiver?
Ans: Yes, we do accept them.
- Do you Accept GRH or SSI for room and board?
Ans: yes, we accept SSI for room and board. We are also in the process for the GRH but are not yet GRH approved/certified.
- Deposit/Application Fee?
Ans: The good thing is we do NOT have a deposit, or an application fee.
Ans: our current addresses are: A-1225 Cook Avenue East Saint Paul MN 55106 & B-2092 Oakridge St, St Paul, MN 55119, but we’re in the process of expanding our locations.
Ans: We have two immediate opening now in 1225 Cook Avenue East Saint Paul MN 55106, but we are expecting multiple (5) openings in our other location on Oakridge St when licensed.
- Waiting List: (size & length of wait time)?
Ans: We currently do NOT have waiting time.
- Forms of payment acceptable?
Ans: Yes, we do accept direct deposit, Check and Money Orders.
- Are you subsidized rent? If so, what is pricing for rent?
Ans: Although we are working on that, but we are not subsidized rent yet, for that reason we cooperate reasonably with our potential residents in a fair manner for all.
- Age Group Served?
Ans: We serve 18 and above in our 1225 Cook Ave location, but in Oakridge it will be 55+yrs when licensed.
- Male or female?

Ans: Yes, we do have placed multiple comfortable chairs outside for our residents to enjoy.
- Pet Policy? Do you allow Cats and/or dogs to visit and stay if sprayed neutered?
Ans: Sadly no, we don’t allow any kinds of pets in the facility, neither visiting nor staying.
- Do you guys do 1-2 person assist?
Ans: Yes, we do 1-2 person assist.
- Do you guys do bathing, dressing, toileting, feeding tube Etc.?
Ans: Yes, we do bathing, dressing, toileting, feeding tube and so on.
- How many Residents in home?
Ans: Our current home is four (4) client rooms only, but the other location will be five (5) client rooms maximum when license is issued.
- Do you have stairs?
Ans: Yes, we do, but our residents with walkers, wheelchairs or are bed-ridden live on the main floor(s). Those residents who have no trouble walking live on other floors.
- Is this home 1 level or 2 levels?
- Ans: Our homes are considered one level homes, but have got downstairs.
- Do you have Stair lifts?
Ans: We don’t have them yet, since we never needed, but we can install them when needed.
- Is your home extra secured?
Ans: Yes, our homes are extra secured with ADT security systems. We have got security alarms and cameras on every exit door, plus the 24/7 awake staff. We are also in the process of putting cameras in all legally allowed places.
- Does anyone speak different language?
Ans: Yes, all our staff are multilingual staff. Most of them speak two or more languages, some even four languages (example: English, Arabic, Urdu, Swahili, and Somali).
- Do you accept clients with Dementia?
Ans: We don’t have a specialized locked unit for Dementia clients.
- Felony friendly?
Ans: Yes, we are felon friendly.
- 24/7 awake supervision?
Ans: Yes, we do have 24/7 awake staff.
- Proximity to bus stop?
Ans: We’re very close to a bus stop, shopping areas, dining areas, and a gas station.
- Transportation for Outings?
Ans: Yes, we do provide transportation for Outings to our residents.
- Do you have a notice period
Yes, we do have a signed 30-days’ notice period if and when the client wants to move out.
