Pinnacle Services provides innovative support to help people achieve their personal goals and improve the quality of their lives. We work with a wide range of individuals and programs, offering flexible services to meet each individual’s needs for design, delivery and level of support. We also bring cultural diversity, person-centered services, high standards and proven quality to the individuals we serve.
Individualized Home Supports with Training
We provide Individualized Home Supports with Training to individuals in their own home and/or out in the community. Services are tailored to the needs of each individual with a focus on obtaining and/or maintaining independence in the community and at home.
Individualized Home Supports without Training and Respite
People with disabilities have the right to control their own lives, including the services they receive. Services for Individualized Home Supports without Training are self-directed programs that offer flexibility in building transparent support plans around individuals and their families. Respite services are provided to people unable to care for themselves, furnished on a short-term basis to provide relief for primary caregivers. Together, these services aim to maximize family strengths and support individuals to live in an environment that is the least restrictive.
Homemaker Services
Homemaker services include general household activities provided by a trained homemaker when a person is unable to manage their home, or when the primary caregiver is temporarily absent or unable to manage the home. These services can include meal preparation, help scheduling appointments, shopping and laundry assistance, and transportation. The goal is to support individuals and help them lead independent, fulfilling lives while remaining in their own home.
Employment Services
We help people with disabilities find meaningful work that aligns with an individual’s skills and aspirations. Working with our community partners, we strive to support individuals in maintaining independence while becoming more connected with the local community. Employment options can be tailored to an individual’s needs and may require an on-the-job assessment to determine the best fit. Our employment specialists will work with the entire care team to find a situation that is safe, supportive and in keeping with an individual’s goals.
Ryan White Services
We are proud to work with those in the Ryan White program, providing necessary medical care, support services and medications for low-income people with HIV/AIDS. Our goal is to improve health outcomes and help those in the program live rich, meaningful lives. We can help individuals navigate program requirements to ensure continuity of care. Our services can be tailored to meet an individual’s unique needs and always deliver services in a respectful and supportive manner.
