24 Hours Community residential settings(crs)

Our homes are located in the nice, safe and quiet neighborhood. Our home is within easy access of public transit, community centers, schools, parks, banks, restaurants, malls, cinemas and other amenities. Rapha Health Care Services offers to clients home like environment, 24- hour supervision and continuous professional care relative to housing, hospitality services and personal assistance so that they can live independently, even as they require regular help with their daily activities. Our community residential services are directed towards meeting the very specific needs of adults recovering from acquired brain injuries, mental illness and other physical and intellectual disabilities. Our Respite home offers an ideal setting for rehabilitation and recovery after injury.

The following areas are taken into consideration: • Communication Training, Time Management, Behavior and Social Skills • Comprehensive Nursing Program, Medication Management • Behavior Management Activities of Daily Living, Personal Organization • Community Participation • Volunteer Programs, Vocational and Educational Pursuits • Computer Skills / Cognitive Exercises • Cooking, Gardening, Outing, Orientation to the Community; includes non-medical transportation.

Physical Disability Support

Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our caregivers can assist you with daily tasks such as mobility, eating, exercising, and grooming.

Experienced and Supportive:

For each service, you select a trusted worker who best suits your needs and maintain control of how your workers provide services by scheduling hours and providing them direction on your specific day to day support needs. Our staff partners with you and your case manager to initiate services, develop and implement your support plan and work to ensure you are receiving a high quality of care. We also provide your worker with 245D training to help both you and your worker feel comfortable and confident in their abilities
