Fraser Community Living Programs

Finding housing can be difficult for individuals with special needs. Maybe they’ve been living at home, but are ready to move out on their own. Or perhaps they have reached adulthood, but still need some support at home. Fraser can help.

Fraser Community Living offers three different living options for adults with special needs: Supervised Living, Supportive Living and Independent Living. These distinct housing options are as unique as the people who live there. Our goal is to provide just enough support to ensure they can live comfortably on their own.

Supervised Living

Fraser has 25 Supervised Living homes available to adults with special needs. These homes are located in neighborhoods throughout Hennepin and Anoka counties. The housing choice is for individuals who need more support at home and to participate in their community.

In Supervised Living, Fraser staff is on-site 24 hours a day. They help residents manage their daily needs and encourage them to work toward goals. Staff helps residents with grocery shopping, meal prep and attending events in the community.

Porcupine

  • Type of home:  Split-level home
  • Location Information:  Located in a quiet cul de sac on the eastern side of Eden Prairie
  • House information:  Upstairs has 3 individual’s bedrooms, the main living area, kitchen and a bathroom with a tub. Downstairs contains one individual’s bedroom, bathroom, laundry area, and a small common area. The downstairs also contains the staff sleeping area and office.  There is one cat in the home!
  • Staffing:  Typically 1:4 with an asleep staff overnight
  • Genders:  Male only
  • Current Age Range: 20s-70
  • Hobbies/Likes in the home:  The individuals like to spend time in the living room watching TV or hanging out.  Things also enjoyed are cardio on the treadmill, going shopping or to the mall, going to the movie theater, going to sports games, etc.
  • Individuals in the home:  All current individuals in the home attend day programs.   It is a higher functioning home.  One individual has a cat, that he takes care of fully.  On weekends, 1 or 2 individuals may go spend time with their families!

Northwood

  • Type of home:  Accessible home with all living areas accessible on the ground floor, as well as ramps by front door and in garage
  • Location Information:  located in New Hope, right off of 169
  • House information: 3 out of 4 bedrooms and 1 bathroom have ceiling lifts.  There is a seated tub in one of the bathrooms
  • Staffing:  2-3 staff 24/7.
  • Genders:  Female only
  • Current Age Range: 40s-50s
  • Hobbies/Likes in the home:  The ladies enjoy Disney movies and music, karaoke, parties, coloring, arts and crafts, puzzles, shopping, activities at the mall, going to the movies or out to eat, etc.
  • Individuals in the home:  Higher functioning home and two individuals use wheelchairs.  Most individuals stay home on the weekend, 2 individuals attend a day program each day of the week, all individuals typically go with their families for holidays or random visits.

Goldenrod

  • Type of home:  Split-level home
  • Location Information:  Located in Plymouth, near Eagle Lake Regional Park
  • House information: Bedrooms on both floors of the home, 2 bathrooms, a balcony in the back with a table and seating for relaxing or enjoying a meal or activity on a nice day, large backyard, and lots of community space for watching TV or doing activities
  • Staffing:  The home is staffed 2:4 and offers awake overnight staffing
  • Genders:  Male only
  • Current Age Range: 50s-60s
  • Hobbies/Likes in the home:  Puzzles, listening to music, sports, going for walks, going out to eat,
  • Individuals in the home:  Social individuals with some moderate intellectual diagnosis.  Individuals attend day programs during the week. Most individuals go on visits with family.

Louise

  • Type of home:   Split-Level.
  • Location Information:  Golden Valley, MN.  Located off Duluth St and Hwy 100.
  • House information: There are 2 bedrooms upstairs and 2 bedrooms downstairs. 
  • Staffing:  The home is staffed Mondays and Tuesdays during the day, but no staff Wednesday-Friday until 1pm.  Normally staffed a 2:4 ratio, but sometimes as 1:4 with asleep staffing at night.
  • Genders:  Female only
  • Current Age Range: 40s
  • Hobbies/Likes in the home:  Board games, crafts such as diamond art, going for walks-especially to the park nearby, watching TV/movies, bowling, word finds and coloring books, church groups, going shopping and out to eat, and going out to the movie theater!
  • Individuals in the home:  2 individuals attend day programs, and all go spend time on outings with friends and/or family members. 

Supportive Living

Fraser Supportive Living offers person-centered services to adults living in their own apartments. Our team specializes in supporting people who have a developmental disability or intellectual disability diagnosis.  Services are based on the unique strengths and needs of each person, with Fraser staff working to help them live as independently as possible. The services Fraser offers to support these individuals can be covered by either a DD or CADI waiver (or private pay at the same rate).

Alcott

Fraser has availability to provide ICS services to individuals living in their own 1 bedroom apartment, or a 2 bedroom apartment with a roommate at the Alcott. Alcott is in Minnetonka, located near Shady Oak RD and walking distance to several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment. County waiver funding is used to pay for staffing and services provided.

Independent Living

Fraser has five Independent Living apartment buildings. These apartments are for adults, 18 years and up, with developmental disabilities who can live independently with little support. Independent Living buildings are located in Apple Valley, Bloomington, Hopkins, Minnetonka and Richfield.

Fraser staff mediate tenant issues, help with emergencies, plan activities and assist residents with engaging in the community. Residents pay 30% of their adjusted gross income in rent.

We currently have no openings in our Independent Living locations, and our waitlist is closed.
