Attentive Care is a group home for individuals 18 years & older who require round-the-clock healthcare services. We exist to serve individuals with physical &/or mental health needs, & can no longer live independently at home. Our services are tailored to meet the needs of each resident. Our available homes are comfortable, safe, & peaceful. Moreover, our dedicated staff are trained in caring for our clients; all while providing them with respect & dignity they deserve. We provide a path continued independence & a happier lifestyle for someone you love. Our Personal care aides & Home Health Aides provide assistance & support with activities of daily living. Our registered & licensed practical Nurses provide skilled nursing care. All of our caregivers have been screened for proper training, qualifications & meet physical requirements.

Assistance with medical and non-medical appointments

Personal care (bathing, dental care, hair care, toileting)
Dressing and undressing

Transferring to and from furniture

Transportation for Individual/Group:
Supports Include:
Taking clients to medical and non-medical appointment
Assisting client with transportation arrangement
Taking to personal and group activities in the community
Mental Health Management:
Services Include:
Ongoing Behavior Management and Monitoring
Dementia and Alzheimer’s Disease Care
TBI -Memory care
And other cognitive/mental health need
Health Related:
Medication administration or assistance with self-administration as delegated by the RN
Verbal or visual medication reminders
Diabetes: Blood Glucose testing and monitoring, Document and report results as requires, follow diabetes diet plans, assistance with physical exercises, Client education, and communicate with healthcare team members as necessary.
Monitor diet & eating habits
Hydration reminders
Light exercise assistance
Other procedures as delegated by the RN
