Fraser Community Living Programs

Finding housing can be difficult for individuals with special needs. Maybe they’ve been living at home, but are ready to move out on their own. Or perhaps they have reached adulthood, but still need some support at home. Fraser can help.

Fraser Community Living offers three different living options for adults with special needs: Supervised Living, Supportive Living and Independent Living. These distinct housing options are as unique as the people who live there. Our goal is to provide just enough support to ensure they can live comfortably on their own.

Fraser has 25 Supervised Living homes available to adults with special needs. These homes are located in neighborhoods throughout Hennepin and Anoka counties. The housing choice is for individuals who need more support at home and to participate in their community.

In Supervised Living, Fraser staff is on-site 24 hours a day. They help residents manage their daily needs and encourage them to work toward goals. Staff helps residents with grocery shopping, meal prep and attending events in the community.

We currently have openings at our Goldenrod, Evelyn, Hansen, Lane, Louise, and Zane, locations:

Goldenrod (male only) is a split-level home located in Plymouth near Eagle Lake Regional Park. The individuals living there are between the ages of 52-63. When COVID restrictions do not prevent it, some of the residents attend a day program and others work in the community during the week. The individuals at Goldenrod are very social and enjoy activities such as puzzles, listening to music, basketball, going for walks, and going out to eat. The home is staffed 2:4 and offers awake overnight staffing.

Evelyn (female only) is a rambler style home located in Edina.  The individuals living there are in their late 30s-50s.  They enjoy watching TV, doing puzzles, coloring, going out to eat, for a drive, or having a picnic at the park.  On the weekends, some of the individuals go on outings with their families, but most of the time they are all home.  The house currently offers 1:4 staff support, with awake overnight staffing.  On evenings and weekends, there is often 2:4 staffing provided.

Evelyn: Evelyn is a rambler style home located in Edina. The individuals living there are in their late 30s-50s. They enjoy watching TV, doing puzzles, and coloring along with going out to eat, for a drive, or for a picnic at the park. On the weekends, some of the individuals go on outings with their families, but most of the time they are all home. The house currently offers 1:4 staff support, with awake overnight staffing. Hansen: Hansen is a split-level home located in Edina with the main level being the top floor.  The females living there are between the ages of 36-58.  Hansen currently serves people with mild/moderate intellectual disabilities with little to no physical limitations.  The ladies like to watch television, go shopping, read books, listen to music, and go out to eat!  They like to spend time chatting together in the den or the living room space.  Hansen is an asleep overnight house and is typically staffed 1:4.

 

Lane: Lane is an accessible home with a ramp and detached garage. The home is located in a busy Brooklyn Park but nestled in a quiet neighborhood backing against the football field of the local high school. The house is currently home to individuals in their 60s- 80’s but has served those younger in the past. We are open to all genders and currently serve male and female individuals in this home. The home is equipped with aerial lifts for those that need help transferring. Others in the home use wheelchairs or walkers. The home has a wheelchair van for use in the community. The individuals vary with schedules of retired to working part time so the home is staffed 24/7 and usually have a ratio of 2:4 staff during the day as well as awake staff on the overnight shifts.

 

Louise:  Louise is a split level home conveniently located off Duluth St & Hwy 100. Two bedrooms are located upstairs and two bedrooms are located downstairs. The residence is currently home to women in their 40s and 50s. The women are active in the community such as movies, shopping, going out to eat, church groups and other community events. The home is staffed Mondays and Tuesdays during the day but there are no staff currently present Wednesday through Friday until 1p. The house is normally a 2:4 ratio but will sometimes work as a 1:4 ratio and asleep staffing at night.

 

Zane: Zane house is an accessible home located conveniently off of Duluth St and Hwy 100. Three bedrooms are located upstairs and one is located downstairs. There are a total of 3 bathrooms in the home. The individuals in the home are active in the community and enjoy going various places such as movies, shopping, going out to eat, church groups and other community events. There is currently no day time staff. The house is normally a 1:4 ratio and has asleep overnight staff. The ladies that live here are very social and enjoy spending a lot of quality time together. When home, they enjoy arts and crafts, puzzles, word puzzles, and reading as well as enjoying a group movie.

Supportive Living

Fraser Supportive Living offers person-centered services to adults living in their own apartments. Our team specializes in supporting people who have a developmental disability or intellectual disability diagnosis. Supportive Living services include a combination of 24-hour Emergency Assistance and Individualized Home Supports with Training. Services are based on the unique strengthens and needs of each person, with Fraser staff working to help them live as independently as possible. The services Fraser offers to support these individuals can be covered by either a DD or CADI waiver (or private pay at the same rate).

We currently have openings at our Alcott, Parkside South, Greenfield, Woodstone, and Parkside North locations.

 
Parkside North:
Fraser has availability to provide services to an individual with their own bedroom in a 2 bedroom apartment with a roommate,  or their own 1 bedroom apartment at Parkside North. Parkside North is in Plymouth and is located near French Regional Park and several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment.
 
Parkside South:
Fraser has availability to provide services to an individual with their own bedroom in a 2 bedroom apartment with a roommate,  or their own 1 bedroom apartment at Parkside South. Parkside South is in Apple Valley and is located near a bus line and several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment.
 
Alcott:
Fraser has availability to provide ICS services to individuals living in their own 1 bedroom apartment, at the Alcott. The Alcott is in Minnetonka, located near Shady Oak RD and walking distance to several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment. County waiver funding is used to pay for staffing and services provided.
 
Greenfield: 
Fraser has availability to provide ICS services to an individual with their own bedroom in a 2 bedroom apartment with a roommate or for an individual to be in their own 1-bedroom apartment, at Greenfield. Greenfield is in Hopkins and is located near a bus line, walking/biking trails and several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment.
 
Woodstone: 
Fraser has availability to provide ICS services at Woodstone to individuals living in their own 1 bedroom apartment home, or a 2 bedroom apartment with a roommate.  Woodstone is in St. Paul and is located near recreation trails, several shops, and is less than 5 miles from the heart of St. Paul.  Individuals pay their own market-rate rent, utilities and living expenses for their apartment.
 

Fraser has five Independent Living apartment buildings. These apartments are for adults, 18 years and up, with developmental disabilities who can live independently with little support. Independent Living buildings are located in Apple Valley, Bloomington, Hopkins, Minnetonka and Richfield.

Fraser staff mediate tenant issues, help with emergencies, plan activities and assist residents with engaging in the community. Residents pay 30% of their adjusted gross income in rent.
