Finding housing can be difficult for individuals with special needs. Maybe they’ve been living at home, but are ready to move out on their own. Or perhaps they have reached adulthood, but still need some support at home. Fraser can help.
Fraser Community Living offers three different living options for adults with special needs: Supervised Living, Supportive Living and Independent Living. These distinct housing options are as unique as the people who live there. Our goal is to provide just enough support to ensure they can live comfortably on their own.
Fraser has 27 Supervised Living homes available to adults with special needs. These homes are located in neighborhoods throughout Hennepin and Anoka counties. The housing choice is for individuals who need more support at home and to participate in their community.
In Supervised Living, Fraser staff is on-site 24 hours a day. They help residents manage their daily needs and encourage them to work toward goals. Staff helps residents with grocery shopping, meal prep and attending events in the community.
We currently have openings at our Goldenrod, Evelyn, 83rd St., and Sweet Briar locations:
Goldenrod (male only) is a split-level home located in Plymouth near Eagle Lake Regional Park. The individuals living there are between the ages of 52-63. When COVID restrictions do not prevent it, some of the residents attend a day program and others work in the community during the week. The individuals at Goldenrod are very social and enjoy activities such as puzzles, listening to music, basketball, going for walks, and going out to eat. The home is staffed 2:4 and offers awake overnight staffing.
Evelyn (female only) is a rambler style home located in Edina. The individuals living there are in their late 30s-50s. They enjoy watching TV, doing puzzles, coloring, going out to eat, for a drive, or having a picnic at the park. On the weekends, some of the individuals go on outings with their families, but most of the time they are all home. The house currently offers 1:4 staff support, with awake overnight staffing. On evenings and weekends, there is often 2:4 staffing provided.
Sweet Briar (male only) is located in a quiet, residential neighborhood near Knollwood Mall. This is a rambler style home with one-level living. Currently, 3 males in their 20s-60s live at the Sweet Briar Home in Hopkins. The individuals enjoy accessing the community by going out to eat, watching TV, going to the movie theater, attending sporting events, and walking or going for bike rides in the neighborhood. The house has 1-2 staffing during awake hours and asleep overnight staffing. There are regular visits from a Registered Nurse. Everything is located on the main floor of the home, except the laundry and food pantry which is located downstairs in the basement.
83rd St. is a co-ed rambler style home located in Bloomington with the kitchen, dining room, living room, and all 4 bedrooms located on the main floor. There are accessible entrances in the front and back of the home, with a patio set in the backyard. The home is located in a busy residential area, conveniently located just a couple blocks away from Girard Lake Park. Currently, 3 females, in their 60’s and 70’s, live in the home. The individuals enjoy arts and crafts, watching TV, going out to eat, and going outside when it is nice out. The house has 1-2 staffing during awake hours and when individuals are home, and awake overnight staffing. There are regular visits from a Registered Nurse.
Fraser Supportive Living offers person-centered services to adults living in their own apartments. Our team specializes in supporting people who have a developmental disability or intellectual disability diagnosis. Supportive Living services include a combination of 24-hour Emergency Assistance and Individualized Home Supports with Training. Services are based on the unique strengthens and needs of each person, with Fraser staff working to help them live as independently as possible. The services Fraser offers to support these individuals can be covered by either a DD or CADI waiver (or private pay at the same rate).
We currently have openings at our Alcott, Parkside South, Greenfield, Woodstone, and Parkside North locations.
Fraser has five Independent Living apartment buildings. These apartments are for adults, 18 years and up, with developmental disabilities who can live independently with little support. Independent Living buildings are located in Apple Valley, Bloomington, Hopkins, Minnetonka and Richfield.
Fraser staff mediate tenant issues, help with emergencies, plan activities and assist residents with engaging in the community. Residents pay 30% of their adjusted gross income in rent.
