Fraser Community Living Programs

Finding housing can be difficult for individuals with special needs. Maybe they’ve been living at home, but are ready to move out on their own. Or perhaps they have reached adulthood, but still need some support at home. Fraser can help.

Fraser Community Living offers three different living options for adults with special needs: Supervised Living, Supportive Living and Independent Living. These distinct housing options are as unique as the people who live there. Our goal is to provide just enough support to ensure they can live comfortably on their own.

Supervised Living

Fraser has 25 Supervised Living homes available to adults with special needs. These homes are located in neighborhoods throughout Hennepin and Anoka counties. The housing choice is for individuals who need more support at home and to participate in their community.

In Supervised Living, Fraser staff is on-site 24 hours a day. They help residents manage their daily needs and encourage them to work toward goals. Staff helps residents with grocery shopping, meal prep and attending events in the community.

Queensland

  • Type of home:  Queensland is a split-level, family style home
  • Location Information:  Located just north of County Rd 24 and West of County Road 101 in Plymouth, just minutes from numerous parks, restaurants, and shops.
  • House information:  The home has 2 floors, with a large community living room on each floor.  There are two bathrooms. The home has a deck in the back with a patio set and grill, and large backyard with a bench swing.  There is a sensory room in the basement.
  • Staffing:  Typically a 2:4 ratio staffing with awake overnight staffing.
  • Genders:  Males
  • Current Age Range: 20s-30s
  • Hobbies/Likes in the home:  Watching movies/TV, playing electronics, taking walks, and going on outings. 
  • Individuals in the home:  Diagnoses include Autism, Mild, Moderate, and Severe Intellectual Disabilities, and PICA.  Home is a higher needs home with low to mild behaviors.

83rd

  • Type of home:  Rambler style home located in Bloomington with the kitchen, dining room, living room, and all 4 bedrooms located on the main floor.
  • Location Information:  The home is located in a busy residential area, conveniently located just a couple blocks away from Girard Lake Park.
  • House information:  There are accessible entrances in the front and back of the home, with a patio set in the backyard. 
  • Staffing:  The house has 1-2 staffing during awake hours and when individuals are home, and awake overnight staffing.
  • Genders:  Female
  • Current Age Range: 50s-70s
  • Hobbies/Likes in the home:  The individuals enjoy arts and crafts, watching TV, going out to eat, and going outside when it is nice out.
  • Individuals in the home:  Disabilities include Mild Intellectual Disabilities and Autism, with low to no behaviors.

Supportive Living

Fraser Supportive Living offers person-centered services to adults living in their own apartments. Our team specializes in supporting people who have a developmental disability or intellectual disability diagnosis.  Services are based on the unique strengths and needs of each person, with Fraser staff working to help them live as independently as possible. The services Fraser offers to support these individuals can be covered by either a DD or CADI waiver (or private pay at the same rate).

Alcott

Fraser has availability to provide ICS services to individuals living in their own 1 bedroom apartment home, at Alcott. Alcott is in Minnetonka, located near Shady Oak RD and walking distance to several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment. County waiver funding is used to pay for staffing and services provided.

Greenfield

Fraser has availability to provide ICS services to individual’s living in their own 1 or 2 bedroom apartment home, at Greenfield. Greenfield is in Hopkins and is located near a bus line, walking/biking trails and several shops. Individuals pay their own market-rate rent, utilities and living expenses for their apartment.

Independent Living

Fraser has five Independent Living apartment buildings. These apartments are for adults, 18 years and up, with developmental disabilities who can live independently with little support. Independent Living buildings are located in Apple Valley, Bloomington, Hopkins, Minnetonka and Richfield.

Fraser staff mediate tenant issues, help with emergencies, plan activities and assist residents with engaging in the community. Residents pay 30% of their adjusted gross income in rent.

We currently have no openings in our Independent Living locations, and our waitlist is closed.
