The Phoenix Residence has openings across the metro, so there’s bound to be a location nearby!

We develop person-centered, quality living experiences for individuals with disabilities. We are a licensed provider that promotes and sustains independent living in 19 homes throughout the Twin Cities.

We help individuals with disabilities enjoy life by helping them perform simple daily functions: meal prep, light exercise, community outings, social interactions, and housekeeping.

We work diligently in supporting individuals with disabilities realize their dreams and live a life of choice! Phoenix respects individual freedom of choice and understands that independence results from successful experiences and increases an individual’s self-esteem and self-confidence.

In order to be able to utilize our ICF/DD services, an individual must:

• Have a diagnosis of DD or a related condition (e.g. Autism, Cerebral Palsy)
• Require 24-hour services
• A person would need to be screened as eligible for ICF level of care by their Case Manager.
• Case Manager needs to complete a screening at the time of admission. This is often also assessed as a part of a person’s MN Choices Assessment and indicates whether they are eligible.
