Yolanda’s Blessings is a Minnesota-based 245D Home and Community-Based Services provider dedicated to supporting individuals with disabilities in living meaningful, independent, and empowered lives. We partner with individuals and families to deliver personalized support that enhances daily living, promotes independence, and strengthens community connections. Our team is dedicated, reliable, and focused on helping each person reach their full potential.

Services We Provide

• In-Home Support Services
Personalized support provided in the individual’s home to help with daily routines, household tasks, personal care reminders, life-skill development, safety monitoring, and overall independence. Our goal is to help each person thrive in the environment that feels most familiar and comfortable.

• Companion Services
Social and emotional support that helps individuals stay engaged and connected. Companions assist with community outings, appointments, hobbies, social activities, and building confidence.

• Respite Care
Short-term care that allows primary caregivers and families to take time for rest, appointments, or personal needs. We ensure your loved one is safe, supported, and well cared for during your break.

• Community Engagement / Community Living Supports
Support that helps individuals participate in meaningful community activities, build relationships, explore interests, and strengthen their sense of belonging.
