Excellent Care Services Inc. is licensed in the State of Minnesota to provide both Home Health Care Services and
Foster Care Services.
We are located in Rochester Minnesota, the Home of Mayo Clinic, the world-renowned Hospital. Currently, Excellent Care Services Inc have openings in their Foster Home
with a brand name DOMINION HOME.
The Home is staffed with a Registered Nurse (RN) and well-trained staff that that help provide excellent care support to our residents.
2. SERVICES:
We provide services for both male and female adults. Our services include mental health and medical health management, 1:1 staffing per client need, Assistance with ADL’s,
Assistance with both medical and psychiatric appointments.

What We do
Excellent Care Services Inc, provides the best Home Health Care services to clients in the state of Minnesota. We are proud to offer our patients the expertise of well trained Licensed Nurses that provide care management in different areas, including Diabetes and Medication management. We also assist Hospice providers, Nursing Homes, Hospitals, Adult Day Care & Assisted Living Facilities with nurses experienced in home health care and qualified Certified Nursing Assistants. Our services are available on a per diem, temp-to-perm, or permanent basis.
Services We Provide
At Excellent Care Services, our PCAs, Homemakers and Nurses provide a variety of In-home care and Live-In care. Our services includes but not limited to:

- Hand-on assistance with transfers and mobility and much more.
Assessing Your Care Needs
Home care begins with a referral from a physician. The nurse is responsible for:
- Assessing the patient’s condition and needs,
- Developing and managing a mutually agreed upon care plan to meet those needs,
- Providing the skilled care required, and
- Offering the patient and family and/or caregiver education and information to maintain the patient’s health, safety and independence.
