Waivered Services at Health Providers, Inc. offers services for individuals that may have TBI, CADI, CAC, and Developmental Disabilities. This will give all residents a chance to live in a wonderful home like environment with services suited to their needs. The homes are set up to resemble a home much like yours and mine. All staff that work at Health Providers are given the necessary training to complete and provide personal care to all people who live at the homes. Health Providers, Inc. will provide laundry-wash, dry and put away clothing, meals-three per day, housekeeping-clean bedrooms, vacuum, dust, change linens, empty trash, bathrooms, routine activities, health and hygiene, medication set up/administration/insulin dependent, medication review, transportation to local appointments, socialization, and self administration of drugs. We will provide three meals a day, specific to the individual diet, afternoon and evening snacks, 24-hour supervision, assistance with grooming, bathing and dressing as needed. Consumers that live in our waivered homes work towards long term goals that their teams come up with on an annual basis. The “goals” turn into programs which the staff at the homes will assist the individuals with on a weekly basis. The consumers living in these homes are working towards independence or maintaining their skills.There are four to five bedrooms in all of the homes. Residents also share a main living area, a kitchen area, visiting areas, laundry rooms, and bathrooms. We recommend that all residents participate in the family style setting during meal times. Activities are done throughout the week to promote socialization. Staff will also assist residents at shopping areas, out for coffee, or whatever they may like to do. Being part of the community is a huge part of all consumers living in our homes. Normalization is promoted daily. Health Providers, Inc. has staff sufficient in qualifications and numbers to adequately provide the services agreed to for Waivered Services and the Adult Foster Care License through the County.
