At Humble Care AFC, we provide home care services to individuals come from a variety of backgrounds including persons with Developmental Disability(DD), Elderly waiver, Community Access for Disability Inclusion(CADI), Alternative Care(AC), and Community Alternative care(CAC). We provide food, shelter, protection, and 24-hour care for eligible vulnerable adults. Individual attention to personal and cultural needs is of high priority. We provide personalized care to meet everyone’s needs with a positive and empowering approach while promoting independence. We provide a safe and welcoming environment that fosters and encourages clients to enjoy life in the community.
Services include:
- 24-hour assistance
- Transportation to and from medical appointments, shopping for grocery, and as well as various activities per resident’s request
- 3 meals per day will be provided with wide range of menu
- Unlimited snacks throughout the day
- Personal care (ADLs)
- Medicine set up and administration
- Laundry service
- Staff accompaniment to doctor appointments
- Shower/personal care assistance as requested
- Each resident will have access to high speed internet and entertainment
- Daily walk to nearby parks, or shopping center per request
- Visitation are permitted with prior arrangements
- Weekend outings per resident request
