Recuperative care services provide short-term support and recovery assistance for individuals healing from chronic illnesses or injuries who do not have a safe place to recover. These programs offer 24-hour shelter and nutritious meals in a secure, supportive environment.
Services include basic medical care, such as wound care, medication management, monitoring of chronic conditions, and coordination of follow-up appointments. Case management and care coordination help clients connect to ongoing medical care and stable housing options after discharge.
Staffed by nurses, case managers, and support staff, recuperative care facilities promote recovery, help prevent hospital readmissions, and support long-term health and stability. Typical stays range from 2 to 8 weeks, depending on individual medical needs.
Recuperative care services are available to eligible Minnesota Health Care Programs (MHCP) members experiencing homelessness to help prevent hospitalizations; or provide medical care and support services when they are unable to recover from a physical illness when living in a shelter or they are otherwise unhoused. MHCP members receive recuperative care services when they don’t need to be hospitalized or remain hospitalized, or don’t meet severity of illness for other levels of care.
Midwest Transitional Housing, Inc. was established in January 2025 to address a critical gap in care for individuals recovering without stable housing. We provide supportive environments for adults ages 18 and older who have been discharged from the hospital but are not ready to return to shelters or the streets.
Our services focus on medical recovery, daily support, and housing stabilization for eligible Minnesota Health Care Programs (MHCP) members. Through collaborative care, individualized plans, and a commitment to dignity, we help individuals heal safely and prepare for long-term stability.
