We specialize in assisted living for individuals with TBI’s, mental/behavioral, and physical health needs. As a certified CADI waiver facility, we accept both CADI waivers and private pay options for our residents. Our home offers comprehensive residential care, ensuring a supportive environment that caters to various care housing requirements.

At our assisted living facility, we have 24/7 on-site unlicensed direct care staff available to meet the needs of our residents. During business hours, an RN/LPN is on-site and available on-call after hours to provide comprehensive care. Our services include medication management, diabetic monitoring and management, and assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs). We also offer transfer assistance, including the use of mechanical lifts, housekeeping, laundry, and linens. Meal preparation and feeding assistance are also part of our care housing services. In addition, we assist with scheduling appointments and managing shopping and finances. With a small staff-to-client ratio, we ensure personalized care tailored to the needs of each client in our home. Our staff also facilitates socialization through in-home activities. We collaborate with Bluestone Physician Services to provide primary medical, mental health, and care coordination services to each resident of our residential care facility. Physicians are available for monthly or quarterly visits and are accessible 24/7 to address any needs at the facility, which helps to prevent or reduce hospitalizations. Furthermore, we offer pharmacy, lab, and imaging services at each location, making us a comprehensive CADI waiver facility.
