Azalea Homes was established with a heartfelt mission: to deliver exceptional care to seniors grappling with various health concerns in a home-like setting. Our fully wheelchair-accessible home  is thoughtfully designed with features like ramps, walk-in showers, and grab bars to support safety and ease of mobility.

We serve adults 55 and older with various health concerns, helping with daily living activities such as bathing, dressing, grooming, medication management, and more. Our trained care team bring over 10 years of experience in both medical and home care settings, ensuring that every resident receives the personal attention they need.

Who We Care For
Azalea Homes proudly serves adults aged 55 and older who are navigating a variety of health-related challenges.

We welcome individuals with:

• Mental and emotional health diagnoses, including depression, anxiety, bipolar disorder, schizophrenia, and other behavioral health needs
• Functional limitations due to age, chronic illness, or disability
• Complex or chronic medical conditions that require ongoing care coordination and support
• Recovery needs following hospitalizations, surgeries, or treatments such as chemotherapy
• Residents needing long-term support in a residential, non-institutional setting

What We Provide
Our highly trained and compassionate team is equipped to deliver a wide range of personalized care services, including:

• Comprehensive diabetic care, including insulin administration, sliding scale management, and blood glucose monitoring
• Medication administration and management, with a focus on accuracy, safety, and education
• Behavioral and mental health support, using proactive strategies to promote stability and emotional well-being
• Wound care and skin integrity management, including pressure sore prevention and treatment
• Assistance with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and ambulation
• Meal planning and nutritional support, including special diets and meal prep assistance
• Household support and home management, including light housekeeping, laundry, and personal shopping
• Social engagement and companionship, to reduce isolation and enhance emotional wellness
• Transportation coordination for medical and non-medical appointments
• Money management support, budgeting, and bill tracking for residents in need
